The format of the address is:

• street address -- type the user's street address in the two text boxes provided.
• city -- type the city name that goes with the street address.
• state -- select the state that goes with the city. You can change the list of states by modifying the state table in the database.
• postal code -- type the zip or other appropriate postal code.
